What is File of Life?
The Chesterfield File of Life program is a communitywide initiative sponsored by Chesterfield Triad to help residents prepare for medical emergencies. This program provides a fillable document where individuals can record important medical information, such as current prescriptions, medical history, and allergies, ensuring that first responders have quick access to details that can save lives. By keeping this information readily available, responders can deliver the best possible care, notify loved ones, and share vital details with hospital staff.
Keep Your Information Accessible
Participants are encouraged to place their completed File of Life on their refrigerator, making it easy for emergency personnel to locate. Free stickers and magnets are available at the Community Engagement and Resources Office, located on the first floor of the Community Development Building, to help identify where the form is stored. The File of Life (PDF) is a fillable document that can be downloaded, printed, and updated as needed.
